Your treatment plan will include a breakdown of all applicable fees, and we will inform you of all costs before treatment is administered. Payment and/or insurance co-pays are due when services are rendered. We accept all major credit cards and debit cards.

A member of our staff will review our payment options with you to find an option that best accommodates your financial needs. This practice serves all patients regardless of ability to pay. Discounts for essential services are based on family size and incomes. If special arrangements are needed, please speak with management.

We accept new patients with all major commercial insurances and all insurances, including Tennessee and Kentucky Medicaid for Obstetric patients. Our office staff will be glad to assist you with your insurance coverage. When you first visit our office, bring your current insurance card with you. If you change insurance companies or join another employer’s plan, please be sure to let our insurance department know. If an insurance referral is necessary, please bring the completed form with you to your appointment.

We will submit all insurance claims for you, doing our best to ensure you receive full insurance benefits; however, you are personally responsible for your account, and we encourage you to contact us if your policy has not paid within 30 days. If we are not contracted under your insurance, you will be given a copy of services and charges to file with your insurance claim form.
